This week, we will begin reaching out to all patients who may qualify to be vaccinated now in order to verify their eligibility. You will have the opportunity to let us know if you live or work in low income or affordable senior housing and tell us about any medical conditions that may not be in our records. You do not need to contact your doctor or hospital for proof of your medical conditions. You will just need to complete a brief form and submit your response. Your answers will help us invite you to schedule a COVID-19 vaccination appointment at the appropriate time in the state’s rollout plan. |
